Yes. If your agency was NOT providing nonmedical home health services before September 30, 2021, you must obtain a $20,000 surety bond from a company licensed to do business in Ohio. This bond requirement is part of the OAC Chapter 3701-60 licensing process.
The nonrefundable application fee for Nonmedical or Skilled Home Health Services is $250. For Hospice Care Programs, the fee is $600. These fees must be paid via cashier's check or postal money order payable to "Treasurer, State of Ohio."
Ohio requires criminal background checks through the Ohio Bureau of Criminal Investigation (BCI) for all employees providing direct care. Primary owners of home health agencies must also complete fingerprinting as part of the license application process under ORC 3701.881.
Ohio home health agency licenses and hospice care program licenses are valid for three years. Renewal applications must be submitted at least 90 days before expiration. Beginning February 1, 2026, ODH provides advance notice of upcoming expiration dates.
